for balance reasons I would rule surprise only. If you start adding that bonus to every attack a journeyman makes after a pommel strike, it gets a bit strong. (granting it as an additional bonus at master might work fine)
From a play logic point of view, I think the follow up would make a ton of sense. If you play test it and see no real disruption from it, go ahead and use it that way.
